Medical Social Worker (Full-Time)
Provide social work assessment and practice with clients and their families with complex emotional or social problems. Facilitate their understanding and resolution of problems related to environmental stress, physical illness and interpersonal conflict. The Medical Social Worker completes patient psychosocial assessments and based on client needs will provide emotional support, brief interventions and education on community resources and appropriate referrals.Read more.
Homeless Outreach Coordinator (Full-Time)
The Homeless Outreach position will build and maintain partnerships with local organizations that expand the reach of EIHC’s mission and our Healthcare for the Homeless grant. The Chief Operating Officer provides primary responsibility for operational management of this complex, multispecialty $12.5MM budget Federally Qualified Health Center. A Masters Degree in Health Care Administration, Nursing, or other related field, with a strong emphasis on practice management is required.
